What are some expected outcomes for a nursing diagnosis of risk for self-harm?


Pt has self inflicted abd wounds and has already pulled out one IV. My book only has "Person will choose alternatives that are not harmful." I've got a couple of others, but would appreciate any other useful input.

We're supposed to have specific outcomes. I have "Pt will not pull out necessary IVs or her foley." But other than that, what else do I say that's specific? All I can think of is, "Pt won't harm self." Which isn't very specific, I don't think?

"PT has expressed thoughts of suicide but seems more desperate to get into therapy to deal with her/his problems."

"Pt has agreed to sign 'no-suicide' contract"
